You can print a Certificate of
Insurance (COI) by entering your medical license number
and PMSLIC policy number.

PMSLIC policyholders can copy their Declarations and Schedule page in order to provide evidence of insurance for credentialing purposes, to apply for hospital privileges, to participate in managed care plans, or other situations where third parties need certification to approve practice activities.
Remember that the original DEC page must be kept in the practice facility or with the policyholder. It provides information regarding limits of liability, codes identifying coverage types and effective dates.
