Payment for Claim Histories

Current insureds, previous insureds and third parties often ask how to go about obtaining claim history reports from PMSLIC. Please refer to the following procedures to ensure a prompt response.

Requests from Current Insureds

  • PMSLIC provides claim history reports to current insureds at no charge.
  • Current insureds can obtain a claim history report online.
    The document should be saved as a pdf and e-mailed to the requester by the current insured.
    Please keep in mind that the information contained in the claim history report is confidential. Sending this report via e-mail could place it at risk for interception and possible transmittal to unidentified parties.
  • Alternatively, current insureds can mail or fax a dated and signed written request using the Claim History Report form located below. PMSLIC will send the claim history report directly to the insured.

Requests from Previous Insureds or Third Parties

  • Previous insureds or third parties must mail a dated and signed written request to PMSLIC.
  • The request should specify to whom PMSLIC should send the claim history report and note the number of years to be included in the report (i.e., 5 years; 10 years; all years, etc.).
